Use this form to opt in to receive short, anonymous civic polling questions from CentPoll by SMS. Your phone number is used only to deliver the surveys you consent to — never shared with third parties and never used for marketing.
Program disclosures
- Program name: CentPoll civic polling.
- What you'll receive: short, anonymous multiple-choice or rating questions delivered one at a time. You reply with the number of your choice.
- Message frequency: varies. Typically fewer than one survey per quarter; during a survey, expect a greeting plus one message per question (usually 3–10).
- Msg & data rates may apply. Check with your wireless carrier.
- Reply HELP for help, STOP to opt out at any time. STOP is honored within 60 seconds across every CentPoll campaign and channel.
- Consent to receive these messages is not a condition of any purchase.

- First message you receive will be the opt-in confirmation, then the survey begins.
Changed your mind?
Visit the opt-out form or reply STOP to any message. Your number is removed from every CentPoll campaign within 60 seconds and kept on the do-not-contact registry for at least four years.
